It's hard (certainly legendary difficulty in TQ is substantially harder than merciless difficulty in PoE), but unlike in PoE you don't feel like the designers tried to shoehorn you into specific playstyles, with the balance being completely changed every three months, voiding all your effort and time you invested in a character or a build. It shares some designs with Path of Exile (which I played 1800 or so hours until the labyrinth was introduced, which really killed the game for me), but is less. In many ways, Titan Quest's fundamental game design is like Diablo 2. In the pre-AE version, I'd do Iron Man characters that just "live off the land" or that only used green items, etc.
